Has your loved one been diagnosed with dementia or Alzheimer's? Feeling lost in front of all the forms and coordination required for treatment? Interested in exercising your rights but don't know where to start?
Family Envelope , a new service of experts from the EMDA Association, provides support and management of the patient's file. The family companion helps the family make decisions on medical, legal and personal issues, and accompanies them in dealing with government institutions and ministries.
What does the service include?
- Reading and accessing medical documents
- Assistance with submitting applications (long-term care benefits, medical cannabis, hospitalization code, and more)
- Building an optimal treatment plan based on the patient's condition and stage of the disease
- Assistance in locating and receiving support from doctors, foreign workers, hospitalizations, and more
