What is a family envelope??
" Family Envelope " is a patient case management and support service. The service is provided by an experienced professional on behalf of the EMDA Association , who personally accompanies the family of the person with dementia at any crossroads or other complex medical event.
The companion integrates the management of the disease or complex event, helps the patient and his family make decisions at junctures of significant medical, financial, and personal decision-making, accompanies them in interfaces with government institutions and offices, and serves as an address for their needs that arise while managing the disease so that they understand and feel that they are not alone in the battle.
As part of a family envelope service , the case manager meets and physically accompanies the patient and his family at home or in the hospital, builds an action plan with them, guides them on how to act with the existing information and how to obtain missing information, provides practical tools that help with daily conduct, and is there for them.
Purpose of the service
Providing guidance, guidance, and support in a variety of areas to family members who are caregivers, with the aim of going through the journey of the disease in the best and easiest way possible.
Within the framework of the family envelope, the patient's case manager provides, among other things,:
- Reading medical documents and assisting in their understanding.
- Assistance in submitting documents and requests on various topics (nursing care benefits, medical cannabis, hospitalization code, etc.).
- Assistance in building a treatment plan according to the patient's condition and stages of the disease.
- Assistance in locating doctors suitable for the patient's needs and geographical location.
- Assistance in locating a hospitalization that meets the needs of the patient and his family.
- This refers to nursing homes.
- Accompaniment and support in obtaining permits for foreign workers.
The service is paid.
1,250 NIS for five hours
250 NIS for each additional hour
