Monoclonal antibody against amyloid β, from Eisai and Biogen.

Patients diagnosed (biologically) as suffering from early-stage Alzheimer's disease (moderate cognitive decline or mild dementia)

Lecanmab is a drug that locates and removes amyloid protein deposits in the brain. The treatment is given by intravenous infusion once every two weeks. Clinical trials have shown a slowdown in cognitive-functional deterioration by an average of 27% over six months compared to the course of Alzheimer's disease in untreated patients.

Not included in the basket.

Lacanem is approved for use by the FDA (starting in 2023) and by the Ministry of Health in Israel.

The drug, and most of the tests to determine suitability for treatment and monitor treatment, are not included in the health basket in Israel as of 2025. A request has been submitted for inclusion in the 2025 basket.

Cost : Estimated at $2,000–4,000 per month. Treatment costs are partially covered by private health insurance under the out-of-pocket medication section.

Donanmab – Donanemab
(Kisanelle)

Monoclonal antibody against amyloid β, from Eli Lilly.

Patients diagnosed (biologically) as suffering from early-stage Alzheimer's disease ( moderate cognitive decline or mild dementia).

Donenumab is a drug that locates and removes amyloid protein deposits in the brain. The treatment is given by intravenous infusion once a month until a PET scan shows that there are no amyloid deposits left in the brain. At this point, treatment with Donenumab is stopped.

Clinical trials have shown a slowdown in functional cognitive decline by approximately 35% compared to placebo for approximately 7–8 months in early-stage patients.

Not included in the basket.

Donenumab is approved for use by the FDA (starting in 2023).

The medication and most of the tests to determine suitability for treatment and monitor treatment are not included in the health basket.

From Mantine – Memantine
(Abixa, Mamox, Namada)

Patients with moderate to severe Alzheimer's disease.

Sometimes given in combination with a drug from the acetylcholinesterase inhibitor group (see above)

Improves cognitive, behavioral, and daily functioning , especially in advanced stages.

Proposed mechanism: Regulation of the glutamate system in the brain. The drug blocks NMDA receptors in a way that corrects excess glutamate activity, thus aiding learning and memory and reducing impairment in normal function.

There are studies showing that adding memantine to acetylcholinesterase inhibitors improves daily functioning in patients suffering from moderate-to-severe dementia, but there are also studies that do not prove this.

Memantine is not included in the basic drug basket . It is partially included in supplementary insurance frameworks.

Health funds sometimes participate in the cost, depending on the supplementary insurance.

Estimated private cost 120–150 NIS per month for a full dose (20 mg per day). With supplementary insurance, the patient pays about 50% of the price. Treatment only under the supervision of a specialist.

Side effects : restlessness, abdominal pain, diarrhea . Prescription and medical monitoring required.

Ginkgo biloba

Cerebonin

Adults with mild cognitive impairment (MCI) or mild dementia.

Moderate beneficial effect on cognitive functions, behavior and daily functioning.

Proposed mechanism : Improved cerebral blood flow and a balancing effect on neurotransmitter systems (glutamate, dopamine, acetylcholine).

Warning : Treatment with antipsychotic medications in patients suffering from dementia increases the risk of stroke, worsening cognitive function, and mortality.

Antidepressants and anxiety

The cost varies between the different medications. Most are generic and inexpensive (20-80 NIS per month). Treatment is only under the supervision of a specialist.

Side effects : Vary between different medications. For example, SSRIs may cause nausea, sleep disturbances, or decreased appetite, while medications like Remeron sometimes cause drowsiness and increased appetite.

Sleep medications

Warning: Benzodiazepines and Z-drugs can cause and worsen confusion and cause falls. It is recommended to use the lowest possible dose for the shortest period of time.

Cannabis

The Ministry of Health supports some of the programs within the framework of day centers for the mentally ill.

In day care centers and nursing homes, treatments are usually included in the monthly cost (sometimes subsidized by the state for eligible families).

Private treatments may cost 150–300 NIS per session.
